Your logbook (whether in Excel or a dedicated app) must contain specific columns to satisfy a DHA auditor.
A. Patient Demographics:
- Patient ID / Medical Record Number (MRN): Crucial for verification. Do not use patient names (for privacy).
- Age: To show the demographic spread.

B. Procedural Details:
- Procedure Name: Use standard terminology (e.g., "Laparoscopic Cholecystectomy").
- Procedure Code (CPT/ICD): DHA auditors love codes. If possible, include the CPT code.
- Diagnosis: Pre-operative diagnosis.
- Role: This is the most critical column. You must define your involvement:
- Primary Surgeon (P): You performed the surgery.
- First Assistant (A): You assisted the primary.
- Supervisor/Teacher (S): You supervised a trainee.
- Observer (O): (Generally does not count toward volume for licensure).
- Approach: Open, Laparoscopic, Robotic, Endoscopic.

Common Pitfalls Leading to Rejection
Even with a perfect template, small errors cause massive delays. Avoid these:
- Rounding Up Dates: If you are logging cases for July 2022, do not round to January 2022. Dataflow will cross-reference your employment certificate dates. If the logbook says you operated in a hospital one month before your official start date, you will be flagged for fraud.
- Inconsistent Naming: If you list "Dr. S. Khan" as your supervisor in one line and "Dr. Shahid Khan" in another, the system assumes different people.
- Missing Minor Cases: Some surgeons only list "impressive" major surgeries. The DHA wants to see the full scope of your practice, including circumcisions, wound debridements, and biopsies.
- Using "Assist" too often: If you list 500 cases but mark "Assistant" for 450 of them, the DHA will classify you as a surgical assistant, not a specialist/consultant.
